EGFR Alterations

EGFR-Activating Mutations:

  • EGFR mutations commonly occur within exons 18 to 21, on EGFR tyrosine kinase domain.
  • The L858R missense mutation on exon 21 is the most commonly occurring missense mutation seen in cancer. Exon 19 in-frame deletion constitutes the other most common aberration. These make up 90% of EGFR-activating mutations.
  • L861Q on exon 21, G719X (X = S/A/C) on exon 18 and V765A, and T783A on exon 20 are the less frequently occurring alterations. Some in-frame insertion/deletion on exon 20 can lead to activation of EGFR tyrosine kinase activity and resistance
